Step-by-Step Guide to Applying for a Home Care Package

Home Care Packages (HCPs) provide government-subsidised support for older Australians who want to stay independent at home. Applying for a package involves several stages, from initial assessment to starting services. Following each step carefully ensures you receive the right level of support without unnecessary delays.

1. Check Eligibility

Home Care Packages are designed for older Australians who need coordinated care at home. Eligibility is usually based on age and the level of support required.

  • Over 65s (or over 50 for Aboriginal and Torres Strait Islander people) may apply.
  • People under 65 may be referred to the NDIS instead.

2. Apply Through My Aged Care

Start by contacting My Aged Care (1800 200 422 or the website). You will be asked about your health, lifestyle, and current support needs. This information helps determine if you need an in-home assessment.

3. Undergo an ACAT Assessment

If eligible, you will be referred for a face-to-face assessment with the Aged Care Assessment Team (ACAT) or Aged Care Assessment Service (ACAS in Victoria).

  • Assessors visit your home to understand your daily challenges and care needs.
  • They may ask about mobility, personal care, medical needs, and home safety.
  • Based on this, they recommend a Home Care Package level (1–4).

4. Receive Your Approval Letter

After your assessment, you’ll get a letter confirming your approved package level. This letter also places you in the national priority queue, where wait times may vary depending on demand.

  • While waiting, you may be eligible for entry-level support under the Commonwealth Home Support Programme (CHSP).

5. Get Assigned a Package

When your turn in the priority queue comes, you’ll receive another letter and referral code from My Aged Care. This means your funding is now available, and you can start looking for a provider.

6. Choose a Provider

With your referral code, contact approved providers in your area and compare services. Look at:

  • Fees and charges (management, care, exit fees).
  • Availability of carers and allied health professionals.
  • Flexibility in tailoring services to your goals.

7. Develop a Care Plan

Once you select a provider, you’ll work together to create a care plan. This plan details how your funding will be used and which services you will receive.

  • Examples: personal care, meal preparation, physiotherapy, occupational therapy, home modifications, or transport support.

8. Start Receiving Services

After your care plan is finalised, your provider will arrange support workers, allied health, or equipment purchases. You’ll receive monthly statements showing how your funds are spent.

9. Review Your Package Regularly

Your needs may change over time. Request a review whenever necessary, and ensure your provider updates your care plan. You can also switch providers if you are unhappy with services.

Conclusion

Applying for a Home Care Package involves several steps: checking eligibility, completing an ACAT assessment, joining the priority queue, and selecting the right provider. With careful planning and regular reviews, you can make the most of your package to live safely, confidently, and independently at home.
